First gate in SMM chain. Detects fatal violations and applies study-type-specific multipliers. Empirical fatal = FATAL_ZERO band (0.0 multiplier). Pre-empirical fatal = FATAL_FLOOR band (0.30 multiplier). No fatal = CLEAN. If study_type is null with fatal, defaults to pre-empirical path (conservative).
